From Treatment to a Men’s Sober Living House in Miami or Hollywood: How the Handoff Works

Discharge PlanningSeptember 20263 min readBy The Foundry Sober Living

The riskiest week in a lot of recovery stories is the one immediately after a programme ends. Tolerance has dropped, the structure has been removed all at once, and the plan — if there is one — is often a phone number written down on the last day. Here is how the handoff into a men's house in Miami or Hollywood works when it is done properly.

Start two weeks out, not two days out

Beds turn over unpredictably, so the man who calls with a discharge date two weeks away gets choices and the man who calls on the morning takes what is open. Two weeks is enough time to speak to three houses, visit at least one, read the rules and the relapse policy, and sort out the money without making the decision under pressure.

What the house will want to know

Expect questions, and take them as a good sign. A serious residence will ask what you have just completed and where, what you are using or were using, what medication you are prescribed, whether you are under any court or probation supervision, what your work situation is, and what your plan for daytime structure looks like. A house that asks nothing is not screening, and a house that does not screen is not protecting the men already living there.

What your clinical team should hand over

With your consent, a discharge summary, current prescriptions, and your continuing-care plan — which outpatient programme, when it starts, and who your contacts are. Sign the releases while you are still in the building; chasing paperwork after discharge is how the first week gets lost. If you are moving from a programme in one county to a house in another, check that the outpatient side moves with you.

Medication, declared up front

Everything prescribed, in the original bottle with your name on it, declared at intake. That includes medication-assisted treatment. Most houses store controlled prescriptions centrally and dispense them, which protects you as much as anyone. Confirm before you arrive that the house accepts your prescriptions — some do not accept certain ones, and it is better to find that out on the phone than at the door.

Pick the county your obligations live in

If probation, drug court or your outpatient programme sits in Broward, a Hollywood house makes the year easier; if they sit in Miami-Dade, a house on that side does. This sounds administrative and it is the single most common reason a placement quietly fails — a long commute to a weekly obligation becomes a missed obligation by month two.

The first 72 hours

Ask the house what happens in them. The good version: an intake test, a walk through the rules, a chore assignment, your first meeting that night, and somebody introducing you to the other men rather than pointing at a room. The empty days immediately after treatment are where the risk sits, and a house that fills them deliberately is doing the main part of its job.

For case managers and families

Three things to verify before you refer or pay: that the residence is certified through FARR, that somebody is responsible in the building overnight, and that the relapse policy is written and not simply discharge. If you need a higher level of care instead, FindTreatment.gov and the SAMHSA helpline on 1-800-662-HELP are the starting points.

Talk to us early

The Foundry takes men straight from treatment into both houses — the Lakehouse in North Miami Beach and the Beachhouse in Hollywood, both men-only and FARR-accredited. Call or text (954) 817-7143 with your discharge date, even if it is a month out. If we are not the right fit we will tell you who might be.

More in treatment versus sober living, the first 30 days, and what happens on the intake call.
